* Families* If everyone is getting the same plan level (Silver, Gold or Platinum) then you will only create ONE ACCOUNT during sign up.
If there are different levels (example: Both Parents want Silver & their child on Platinum for orthodontics) an account will need to be created for EACH LEVEL. In this case, there would be 2 accounts created. One account for both parents on the Silver plan & one account for the child on the Platinum plan.
Additional Member Pricing* To receive the additional member pricing, all members MUST be on on the same level. The additional member pricing DOES NOT apply across multiple levels.
HSA/FSA/CareCredit/Prepaid Cards are not accepted as Membership payments
Extensive periodontal treatment, endodontic treatment, or oral surgery may be referred to a specialist.
